to the repair scenarios, we pretty much have to rebuild the whole
bin/bii entries on invoice creation for a given account.

of the possibility of bundle transfers between accounts, it's a bit of effort to
associate a row in bos back to a particular account. Add account_key in
bos, bst_tags and bst_fields to work around this.
