to take an additional date (and therefore be able to specify both entitlementEffectiveDate and billingEffectiveDate).
There are still some tasks left to complete the feature (and some related TODO in the code, but this is the bulk when apis have been changed, new BlockingState events
are sent at creation times and all tests modified accordingly)

depending on the reference time,
we might end up in a "gap" where a local time does not exist because of DST.
In this case, use toDateTimeAsStartOfDay() as suggested by the Javadoc of
IllegalInstantException.
