of InternalCallContextFactory
An internal slow test was injecting InternalCallContextFactory before MySQL was
started, and a Finalizer thread was trying to open connections.
Work around this scenario by creating the CallContextSqlDao on demand.

should be priming caches only when the system is ready, not in the
constructor call. Also, the multi-tenancy story is unclear.
