invoice: treat full item adjustments like repairs This fixes …

a bug where the wrong tree ended up being built.

Expectation in testMaxedOutProRation had to be changed because
it was incorrect: item adjustments shouldn't change the expected
view as seen by subscription.

This fixes https://github.com/killbill/killbill/issues/640.
