invoice: CBA improvements Ignore CBA from DRAFT invoices …

and don't rebalance until
the invoice is COMMITTED.

Rewrite CBADao to avoid unnecessary DB queries. Specifically,
implement getAccountCBA in pure SQL to avoid fetching all invoices
when there is no CBA to rebalance.
